This template out-of-the-box is not valid AMP HTML. You'll need to edit CSS to remove !important, plus this is nothing like the demo. The design is great, code not as much. Plus, amp-form has long been out of experimental but this template form will need to be redone to take advantage of that. I wanted to first contact the developer about these issues but the "contact support" link is dead.
